Requesting COBRA Coverage

In requesting COBRA coverage, you must go online to "verizon.com/benefitsconnections" and request COBRA coverage from the company.They will send you a confirmation letter. You give the Dr. your insurance card & the confirmation letter. The Dr. will give you a bill. You then submit the bill to the Union for payment.
